- Candida Adherence Mycology Research Unit.
- Adherence to epithelial cells is the first step in colonization by Candida and subsequent establishment of mucocutaneous infection. Similarly, adherence to intravascular structures is considered to be a critical step in the egress of the blood-borne fungi from the intravascular compartment, as they hematogenously infect target organs. Because the mortality rates associated with candidal infections remain disturbingly high despite the presence of antifungal agents with excellent in vitro activity against Candida, optimal therapy requires strategies to increase host resistance to candidal infection (such as blocking adherence), combined with the use of antifungal agents. ... One of our primary objectives in forming a Candida Adherence Mycology Research Unit is to bring together a group of experienced investigators with long standing expertise in the area of cell to cell recognition to move the field of candidal adherence into a new level of understanding that would not otherwise be possible. ...

- Adherence in depressed HIV-positive patients improved by antidepressant treatment .
- Adherence to antiretroviral therapy is improved by antidepressant treatment in depressed HIV-positive patients, according to the results of a retrospective cohort study published in the 1st April edition of the Journal of Acquired Immune Deficiency Syndromes. The study also found that adherence was highest in patients who were also adherent to their antidepressant medication, leading the authors to call for greater attention to the diagnosis and treatment of depressive illness by healthcare workers. ... Untreated depression has been associated with reduced adherence to antiretroviral medication. ... To assess the effect of antidepressant treatment on adherence in depressed patients, investigators from Denver, Colorado analysed the medical records and pharmacy files of 1713 HIV-positive patients seen at an urban clinic between January 1997 and December 2001. “A high prevalence rate of depression was observed among HIV-infected individuals, for whom antiretroviral therapy adherence improved after prescription of antidepressant therapy,” write the authors, “and reached even higher levels among individuals who were adherent to antidepressant therapy. ... The investigators estimated adherence to antiretroviral therapy by measuring the frequency of refills at the clinic pharmacy. Of those receiving antiretroviral therapy, the mean adherence rate was 72%, with 210 (26%) having optimal adherence rates of more than 95%. Adherence was higher in patients who were taking antidepressant medication compared to those who were not (65 vs. ... For antidepressants, ‘adherence’ was defined as a rate equal to or greater than the median value. Adherence to antiretrovirals improved in the six months following the prescription of antidepressant therapy (p In a multivariate analysis, non-adherence to antiretroviral therapy was found to be associated with non-adherence to antidepressant drugs (p = 0. ... “Our analysis supports the importance of routine assessment of depression, prompt initiation of antidepressant therapy where indicated, and monitoring of antidepressant therapy adherence status,” state the investigators.

4. WHO | ADHERENCE TO LONG-TERM THERAPIES: EVIDENCE FOR ACTION
- www.who.int
- printable version ADHERENCE TO LONG-TERM THERAPIES: EVIDENCE FOR ACTION.
- Adherence to therapies is a primary determinant of treatment success. Poor adherence attenuates optimum clinical benefits and therefore reduces the overall effectiveness of health systems.
- In developing countries, when taken together with poor access to health care, lack of appropriate diagnosis and limited access to medicines, poor adherence is threatening to render futile any effort to tackle chronic conditions, such as diabetes, depression and HIV/AIDS.
- Intended for policy-makers, health managers, and clinical practitioners, this report provides a concise summary of the consequences of poor adherence for health and economics. It also discusses the options available for improving adherence, and demonstrates the potential impact on desired health outcomes and health care budgets. It is hoped that this report will lead to new thinking on policy development and action on adherence to long-term therapies.

5. Enhancement of Treatment Adherence Among Patients With Bipolar Disorder -- Sajatovic et al. 55 (3): 264 -- Psychiatric Services
- psychservices.psychiatryonline.org
- Enhancement of Treatment Adherence Among Patients With Bipolar Disorder .
- OBJECTIVE: Because about one-third of persons with bipolar illness take less than 30 percent of their medication and because nonadherence is associated with rehospitalization and suicide, the literature was searched to identify controlled studies of enhancement of treatment adherence among persons with bipolar disorder. ... Those selected for review were controlled trials that used samples of adults with a diagnosis of bipolar disorder and that measured adherence to either mood-stabilizing medication or psychotherapy. Information was extracted on the diagnostic composition and size of the study group, the type and duration of the intervention, the method of measuring adherence, and outcomes. ... Although the literature on enhancing treatment adherence among persons with bipolar disorder is limited, the existing data are promising and demonstrate development over time in our understanding of how best to manage this illness. ... CONCLUSIONS: Adherence to treatment for bipolar disorder may be enhanced by interventions that address issues of appropriately taking medications to manage illness. For optimum outcomes, promotion of adherence must be integrated into the medication management of bipolar illness. ...

- Context Low adherence with prescribed treatments is ubiquitous and undermines treatment benefits. ...
- Objective To systematically review published randomized controlled trials (RCTs) of interventions to assist patients' adherence to prescribed medications. ...
- Data Sources A search of MEDLINE, CINAHL, PSYCHLIT, SOCIOFILE, IPA, EMBASE, The Cochrane Library databases, and bibliographies was performed for records from 1967 through August 2001 to identify relevant articles of all RCTs of interventions intended to improve adherence to self-administered medications. ...
- Study Selection and Data Extraction Studies were included if they reported an unconfounded RCT of an intervention to improve adherence with prescribed medications for a medical or psychiatric disorder; both adherence and treatment outcome were measured; follow-up of at least 80% of each study group was reported; and the duration of follow-up for studies with positive initial findings was at least 6 months. Information on study design features, interventions, controls, and findings (adherence rates and patient outcomes) were extracted for each article. ...
- Forty-nine percent of the interventions tested (19 of 39 in 33 studies) were associated with statistically significant increases in medication adherence and only 17 reported statistically significant improvements in treatment outcomes. ...
- Conclusions Current methods of improving medication adherence for chronic health problems are mostly complex, labor-intensive, and not predictably effective. The full benefits of medications cannot be realized at currently achievable levels of adherence; therefore, more studies of innovative approaches to assist patients to follow prescriptions for medications are needed. ...
